SALEM TOWNSHIP, Warren County, Ohio — One man died and two others were injured in a June 25 crash at U.S. 22/3 and Middleboro Road.
According to the Ohio State Highway Patrol, the Warren County Communications Center received a 911 call at 11:24 a.m. reporting the collision.
Troopers said a northbound 2004 Toyota Camry on Middleboro Road failed to yield and was struck by a westbound 2012 Dodge Ram 1500 on U.S. 22/3.
Authorities said the Dodge went off the right side of the road, reentered the roadway and stopped; the Toyota went off the left side.
Lebanon EMS took the Toyota's driver to TriHealth Bethesda North Hospital with serious injuries, according to the Highway Patrol.
CareFlight flew the Toyota's passenger, Jerrold Rajewski, 87, of Blanchester, to Miami Valley Hospital, where he died, authorities said.
Hamilton Township EMS took the Dodge's driver to TriHealth Bethesda North Hospital with injuries that were not life-threatening, according to the Highway Patrol.
The Lebanon Post of the Ohio State Highway Patrol is investigating.
