91-Year-Old Driver Dies in I-15 Crash with Box Truck in Midvale.

MIDVALE, Utah (KUTV) — A 91-year-old man died following a crash involving a box truck on Interstate 15 on Monday afternoon.

The incident occurred at approximately 1 p.m. near 8200 South, according to Utah Highway Patrol. Traffic was slowing as a box truck had been pulled onto the right shoulder of the southbound lanes.

Grant Smith Fairbanks, a resident of Cottonwood Heights, was driving a white Forte when he collided with the box truck.

Authorities reported that life-saving measures were attempted on scene, and Fairbanks was transported to a local hospital in critical condition; he later succumbed to his injuries.

The driver of the box truck was not injured. The right two lanes of I-15 were closed temporarily for investigation and debris removal.

Utah Highway Patrol stated that the investigation into the crash is ongoing.
