LEBANON, Tenn. (WTVF) — A human error led to the Wave Swinger ride crash at the Wilson County State Fair on August 12, injuring 9 and sending 4 to the emergency room, state officials confirmed.
According to an investigation by Worldwide Safety Group, the operator initiated an emergency stop but switched the ride back to manual mode before it had fully stopped rotating.
The ride’s contractor, Reithoffer Shows, stated the operator took action after witnessing young riders entangled in chains, causing concern for potential injuries. The company described this as a “moment of panic.”
The emergency stop mechanism is designed to gradually lower the ride, preventing dangerous conditions during its descent.
Witness Tim Nowak reported that the ride was lowering too quickly, stating, “It was just coming down a little bit faster,” while his wife Jamie expressed alarm as the swings remained outward during the descent.
In the aftermath of the incident, the ride was shut down for investigation, and the operator involved will not return to duty. The rides resumed operations on August 21, 2026, for the fair’s final days.
Authorities added that state guidelines mandate all ride operators demonstrate competency, be at least 16 years old, and operate only one ride at a time while remaining present during operation.
