**BOSTON —** Surveillance video released Thursday shows an 81-year-old driver entering Interstate 93 in the wrong direction shortly before deadly crashes on June 27.

The footage, obtained by NewsCenter 5, captures Antone Carvalho of Somerset turning onto Frontage Road from the Mass Ave ramp. The vehicle, after leaving the view of the first camera, is observed merging onto I-93 northbound incorrectly.

Authorities stated that two vehicles attempted to avoid Carvalho's , resulting in a crash involving a family of four in a Odyssey. Carvalho then collided head-on with a Cruze, leading to the death of a Haverhill man in his 20s. Carmavalho sustained unspecified injuries and was transported to a local hospital.

Authorities plan to summon Carvalho to court to face charges. This incident marks the second fatal wrong-way crash in Massachusetts within three days, following the death of Sheena Grandpre, 43, in a separate incident on I-290 East.

In an effort to improve safety, Massachusetts Governor Maura Healey recently announced an expansion of a wrong-way driver detection system, developed following several fatal incidents involving wrong-way drivers, including the deaths of a State Trooper in May and two others earlier this year.

What to do if you see a wrong-way driver:

  • Stay out of the left lane
  • Drive in the right lane
  • Get off the highway
  • Call 911
