OKFUSKEE COUNTY, Okla. (KOKH) — A fatal crash occurred on westbound I-40 in Okfuskee County late Tuesday night, resulting in one death.
Emergency crews responded around 10 p.m. on August 2 to the scene of the accident, involving two vehicles. According to investigators, one vehicle departed the roadway to the left, striking a guardrail before coming to a stop facing eastbound in the westbound lanes.
The vehicle was subsequently struck head-on by a second westbound vehicle. Authorities confirmed that 21-year-old Thi Phan, a passenger in the first vehicle, died from her injuries.
The Oklahoma Highway Patrol is investigating the incident. No other injuries have been reported, and there were temporary lane closures during the response. Further updates will be provided as information becomes available.
